Anyone can refer a patient to Gateway to Hope.

• A biopsy confirmed diagnosis of breast cancer or a medical history indicating an individual is genetically at high risk for developing breast cancer. 

–AND– 

• A family income between 200 percent and 350 percent of the Federal Poverty Guidelines.
